Advance Health Directives
Advance Health Directive ("AHD")
An AHD is a document that contains your decisions about future medical treatment or health care. You can give or withhold consent for specific medical, surgical or dental treatment or other health care. Like an EPG, an AHD can only be used when and if you become unable to make a treatment decision for yourself.

The guide includes a form which you can complete yourself. However, if you would like us to assist, please contact us or download the AHD instruction sheet from this page, complete it and send it to us and we will prepare an AHD for you after giving you a quote.
